• Tattoos: Only permitted for clients 18 years of age or older, with valid government-issued photo ID. No exceptions.

• Piercings: Clients must be 18 years of age or older, OR under 18 with a parent/legal guardian present. Parent/guardian must provide valid government-issued photo ID and proof of relationship (such as a birth certificate or legal documentation).

If getting a tattoo, I have looked over my design, checked spelling if applicable, and give my full consent to the application of my tattoo.*

I acknowledge that I am not pregnant.*

I do not have any undisclosed medical conditions (such as diabetes, hemophilia, skin conditions, epilepsy, etc.) that may affect healing or increase risks.

I understand the risks of tattoos/piercings include, but are not limited to: infection, scarring, allergic reaction, swelling, fainting, dissatisfaction with results, or rejection/migration of jewelry/ink.*

I request a tattoo and/or piercing to be performed on me today.

I confirm that the design, spelling, placement, and/or jewelry choice has been verified by me before the procedure begins.

I understand tattoos are permanent and removal may cause scarring or require costly medical procedures.

I agree to follow the written and verbal aftercare instructions provided to me by the artist/piercer.*

I release Mystics Tattoo & Piercing, its owners, artists, piercers, and employees from liability for any complications, infections, or undesired results from my procedure.

I understand that no guarantees have been made regarding the outcome of my tattoo or piercing.

I consent to photographs of my tattoo/piercing being taken for records and/or shop use (without personal identifiers unless I give additional written consent).

I acknowledge that there is a chance I might feel lightheaded, dizzy during or after being tattooed. I agree to immediately notify the practitioner in the event i feel lightheaded, dizzy and or faint before, during or after the procedure.*

I acknowledge that security cameras are in use within the shop for safety, emergency, and legal purposes only.*
